In a Campeonato Candango match between Brasiliense and Formosa he dished out two yellow cards to the same player (Formosa defender Kassio) but forgot to issue a red.
The first foul took place on 29 minutes and seven minutes later Kassio committed another yellow card offence.
Graham Poll is advocating leniency - saying "It could happen to anybody"
